Fund the creation and provision of educational programs for all levels of clinicians and clinicians in training to help them adopt and implement goal-oriented care

Encourage and support modifications in primary care residency certification and review criteria to promote interdisciplinary team training models

Support development and funding of educational degree programs for primary care administrators

Support goal-oriented training for nurse practitioners and physician assistants

Develop and test a comprehensive primary care logic model that 

can guide future research and policy decisions

Encourage the development and implementation of primary care quality indicators that are process and goal-oriented

Encourage guideline developers to include information on size of impact of recommendations on survival, quality of life, G&D, and QOD

Encourage and support the development of electronic record interfaces and decision-support aids designed to help clinicians and patients document and track progress toward achievement of the four categories of goals

Make sure that new payment models adequately support the primary care functions, goal-oriented care, continuous quality improvement, and investments in new equipment, methods, and other innovations.

 Encourage development of research methods and studies that allow for individualization of interventions (e.g. processes) and outcomes (e.g. pre-specified)

Simplify process for clinicians to include all levels of prevention (including chronic disease management) during Medicare Annual Wellness Visits

Encourage and support comprehensive Annual Wellness Visits for Medicaid enrollees

Support further development, dissemination, and use of comprehensive health risk assessment and other clinical modeling tools to help guide preventive care planning

Improve the accuracy of cause of death determinations by encouraging more virtual  autopsies and better training of clinicians and public health officials

Support long-term studies of the comparative effectiveness of preventive strategies including studies of how to prevent future disability

Incentivize implementation of high-impact preventive strategies

Encourage and support implementation of evidence-based IT interventions to support behavior change

Reward comprehensive care planning that includes clarification of needs, values, and  priorities prior to deciding on interventions

Encourage further development and use of tools to help patients and clinicians clarify patient needs, values, and priorities

Through department development and enhancement grants, encourage residency curricula for family physicians and general internists in human growth and development across the lifespan and for general pediatricians in resiliency development

Encourage and support efforts to develop better ways for primary care clinicians to assess, address, and track multidimensional growth and development across the lifespan

Encourage advance directive discussions during Medicare and Medicaid Annual Wellness Visits

Support reintegration of primary care, urgent care, home care, long term care, and palliative care
